This is used because you are required by Alberta OHS Legislation Code Part 2 requires employers to perform hazard assessments to identify, document, and communicate existing or potential hazards on the work site.

You must complete this card at the beginning of the day, as the scope of work changes, and/ or as the work site changes.

All workers must be involved. Some companies prefer to have each worker do an individual FLHA each day, but can be done by groups of workers if applicable.
